Most of the hypertension therapy demonstrate uncontrolled blood pressure. One of the most prescribed drug for this patients is diuretic (either single or combination), which cause electrolyte disturbances. The aim of this study is to evaluate the effectiveness of therapy and adverse drug reaction of diuretic (HCT) and its combination in hypertensive patient at special clinic of RSUP Dr. M. Djamil Padang. This prospective observational study involve 20 patient which were divided into five groups based on antihypertensive drugs are prescribed, namely group I (only HCT), group II (HCT and 1 other antihypertensive), group III (HCT and 2 other antihypertensives), group IV (1 non-diuretic antihypertensive), and group V (2 non-diuretic antihypertensive). Blood pressure and heart rate of patients are measured on days 2, 5, 7, 10, and the 15th day after the patient went to the hospital and patients complaints during treatment were also evaluated. Blood pressure and heart rate data were statistically analyzed by 2-way ANOVA followed by Duncan’s Multiple Range Test, while patients complaints during therapy analyze descriptively. The results showed that the group I, IV, and V demonstrated the effective of therapy (blood pressure <140/90 mmHg) while groups II and III, eventhough patients blood pressure was reduced but still out of target therapy (blood pressure >140/90 mmHg). Heart rate of all patients were in normal range. Most of the patients receiving HCT complained of fatigue, muscle aches, and headaches. Some patients in the group complained of muscle cramps, dizziness, and nausea. Groups that do not use HCT had almost no significant complaints. These indicated that the treatment of hypertension with HCT and its combination are effective but produce some adverse drug reactions.
